The Institute of Internal Auditor's (IIA's) web site offers several internal control products, including COSO products.
The AICPA sells the original 1992 four volume COSO set titled "Internal Control - Integrated Framework".
If you are a member of the AICPA, the set is only $26.40. Volume 3, "Evaluation Tools," includes a 40-page evaluation guide, with questions that will delve into a company's control environment, risk assessment, control activities, information and communication, and monitoring, along with worksheets. Volume 3 also includes a 90-page reference manual, and a 70-page sample filled in evaluation.
